D0 Full Authorship Application

For the person being proposed for D0 Authorship, please answer the
following questions. See the D0 Authorship Rules for the definitions
of Run 2 Service Tasks (a)-(f).


a) What percentage of research time has been spent on D0?
   What are the dates that this time has been spent?

b) Of the time spent on D0, what percentage was spent on D0 Run 2
   service tasks (a)-(f)?

c) What are the specific Run 2 service tasks (types (a)-(f)) worked
   on? Please give details of the service work.

   For EACH task, please list:
     i)   dates that this service work was done
     ii)  a rough estimate of the amount of time spent on the task
          (average hours/day or fraction of time for each period)
     iii) name(s) of D0 collaborators who can verify the service work.
          It's best if these are people from outside of the applicants
          home institution, group conveners and the like.

d) How many total hours/week was spent on Run 2 service tasks
   (a)-(f) while on D0? (This can be averaged over the year).
   This is the average per week of the times listed in c) above.

e) What is the position of the person? (i.e. grad student, postdoc,
   professor, etc)

      that is in support of a single analysis or physics group.
