D0 Full Authorship Application                 (revised 14 Jan 2012)

For the person being proposed for D0 Authorship, please answer the
following questions. See the D0 Authorship Rules for the definitions
of Run 2 Service Tasks (a)-(f).

1) What fraction of total time is research time?

2) What percentage of research time has been spent on D0?
       (100% unless on more than one experiment)
   What are the dates that this time has been spent?

3) What are the specific Run 2 service tasks (types (a)-(f)) worked
   on? Please give details.

   For EACH task, please list:
   i)   Dates that this service work was done
   ii)  A rough estimate of the amount of time spent on the task
        (total hours or fraction of *total* time for each period)
        *** PLEASE SPLIT THE TIME ESTIMATE INTO *BEFORE* AND *AFTER*
        1 OCT 2011. The requirements differ for the two periods.          
   iii) Name(s) of D0 collaborators who can verify the service work.
        (It's best if these are people from outside of the applicants
         home institution: group conveners and the like.)

4) How many total hours were spent on Run 2 service tasks (a)-(f)
   (This is the sum of the times listed in 3) above.
    This needs to be roughly 1000 hrs reduced by the fraction listed
    in item 1) but must be at least 12 hrs/week averaged over the
    time period listed in 2).)

5) What is the position of the person? 
   (i.e. grad student, postdoc, professor, etc)


NOTE: please do *NOT* include time spent on analyzes or work
      that is in support of a single analysis or physics group.
