Look for the request using the "Show all requests pending approval". If the coordinator is listed as d0-release-mgr@fnal.gov, or if the wrong person has been assigned as coordinator for this package, send a message to the release system administrator.
If you see that you are in fact listed as coordinator, but with a different email address than what you used to register with the system, you can either change your registered email address using the "Customize" button, or request the email address assigned to you as coordinator be changed by sending a message to release system administrator. The system uses your email address to identify you -- your registered address must match the address assigned to you as coordinator.
Not all packages are initially assigned coordinators. By default, these packages are assigned to the release managers. This ensures that all release requests will be processed (the release managers will see such requests).

The system has a limit of 4000 characters for release notes and special instructions. If you get this error, make sure you have less than 4000 characters.
The system uses the CVS command cvs history -Ta to get a list of available tags. If the version you are looking for is not listed with this command, it probably means you used "tag" instead of "rtag". You should retag your package with the rtag command.
The system takes the URL-path and the systems (packages) filename that you specified when you first registered, concatenates them, and then downloads the file from the web. The specified address must be a valid web address. To check, use a web browser and check the url. If you are able to see the file with the browser, report the problem to the release system administrator. You can check (and modify) your URL-path and packages / systems files by using the Customize button.
